HEALTH
HEALTH AND SAFETY:
Screening tests are given students each fall for speech, hearing and vision, with individual follow-up screening throughout the school year, as needed.
EMERGENCIES AND FIRST AID:
State law limits the first aid we can give your child here at school. In case your child becomes ill or receives an injury, you will be notified immediately. If your child appears to be ill in the morning, please do not send him/her to school, but contact your physician for diagnosis.
PLEASE KEEP YOUR TELEPHONE AND EMERGENCY CONTACTS
CURRENT IN OUR FILE!
MEDICATION:
No medication can be given at school unless it is for a term greater than 15 consecutive school days. The school district has a medication policy and your doctor must sign the district medication form before medication can be given. A copy of the policy is available in the school office. All medication must be properly labeled from the pharmacy containing the following information:
- Name of student
- Name of medication and dosage
- Directions for giving medication
- Name of physician prescribing medication
- Date medication was prescribed
After the parent and child's doctor sign a request form for giving medication at school, the medication can be given to the student. The medication will be kept locked by the school nurse.
IMMUNIZATION:
ALL STUDENTS MUST HAVE, BY LAW, A COMPLETE AND UP-TO-DATE IMMUNIZATION RECORD BY THE FIRST DAY OF SCHOOL OR THEY CAN NOT ENROLL.
